Announcement Request Form

The purpose of this form is to collect information about upcoming events at Family Church so they can be announced to the church body.

As a general guideline, if the event relates to less than 50% of the church body, it will be at the discretion of church leadership if it will be announced from the pulpit (other avenues may be available and will be discussed if this arises).
